Before you can do the HCG Diet you must complete the following:

1. Go to our “Intake Form” and fill it out completely 
2. Once you complete the last page of the form and click on “Get Results” we will contact you using the information that you provide on the form.
3. An appointment will be set for either a phone consultation or an office visit depending on how far away you live from our clinic.
4. Your Intake Form results will help to determine what factors may have to be addressed before starting the HCG Diet and a custom program will be suggested for you to obtain optimum results.

You may call to let us know that you have filled out the Intake Form and set your appointment to get started sooner.

 If you call to setup an appointment but have not filled out the intake form, you will be asked to go back to the website and fill it out ahead of time before your appointment will be scheduled.

Consultations usually last about 1 hour.
